We will deliver services to children and young people with additional support needs and to adults with learning disability and autism that meet eligible need and reduces inequality.
The Council is undertaking a Data Protection Impact Assessment with a view to introducing digital tools for young people, parents/carers and professionals to support more effective transition planning. These tools will be embedded into local guidance, and will enable us to collect data regarding individual experiences of transitions locally. Regular meetings have been established between officers in Children's Services and Community Health & Social Care to support improved planning and resourcing.
