Care
The aim of the service is to empower and to improve the quality of life of individuals with mental health illness including supporting their independence and social skills through compassionate care. The service is tailored to individual assessed needs and can be offered on a short-term or long-term basis dependent on assessed need.
If the individual lives in a remote location support may be provided through the Care at Home Service with advice from the team at Annsbrae.
The service meets the needs of individuals with mental health difficulties that need assistance in any of the following areas:
• To improve coping/functioning skills
• To develop/enable activities of daily living
• To manage their finances to the maximum of their ability
• To access community facilities
• To deal with practical issues in the event of crisis
Key workers and Care Plans
Each individual using the service will be allocated a key worker whose role is to be the main point of contact for the individual and their family. The key worker will also be involved in creating and updating a care plan for the individual, which include documents such as a daily support plan and risk assessments and will include as much information as possible about the individual’s likes, dislikes and life history. Family involvement is very much encouraged in the creation of care plans in order for the service to learn as much as possible about the individual using the service.
Staff
All social care workers are registered with the Scottish Social Services Council (SSSC) and have an appointed Senior Social Care Worker who offers support and supervision, identifying training and development to ensure staff are confident and competent in their role. Staff can assess and use equipment to enable you to live safely in your own home.
